Rajesh Ramesh competed in the Indian Under-20 Championships held in Coimbatore, Tamil Nadu in April 2018 and won bronze in the 400 metres sprint. He came 4th in the 400 metres sprint in the South Asian Under-20 Championships held in Colombo, Sri Lanka in May 2018. In June 2018, he came 6th in the 4 X 400 metres relay race at the Asian Junior Championships held in Gifu, Japan. In July 2018, he competed in the 4 X 400 metres relay race at the IAAF World U20 Championships held at Ratina Stadium, Tampere, Finland; however, the team could not move past the first round. He won gold in the 79th All- India Inter-University Championships held in Moodbidri, Karnataka. In August 2021, he won gold in the 400 metres race in the Railway Selection Trials held in Lucknow, Uttar Pradesh. In September 2021, he competed in the National Open Championships held at Stadium Hanamkonda in Warangal, Telangana; however, he could not qualify for the second round in the 400 metres sprint and came 4th in the 4 X 400 metres relay race. In April 2022, he won gold in the 400 metres race in the National Federation Cup held at CH Muhammed Koya Stadium in Thenhipalam, Kerala. In June 2022, he came 4th in the semifinal round of 400 metres sprint and won gold in the 4 X 400 metres relay race in the National Inter State Senior Athletics Championships held at Jawaharlal Nehru Stadium in Chennai, Tamil Nadu. In July 2022, he competed in the 4 X 400 metres relay race in the World Athletics Championships held at Hayward Field in Eugene, Oregon, US; however, the team could not qualify for the second round. He suffered an injury and was selected but later removed from the Indian squad for the 22nd Commonwealth Games held in Birmingham. In October 2022, he won silver in the 400 metres and bronze in the 4 X 400 metres relay race in the 36th National Games held in Gujarat. He won gold in the 400 metres sprint and gold again in the 4 X 400 metres relay race in the National Open Athletics Championships held at Sree Kanteerava Outdoor Stadium in Bengaluru, Karnataka.
